Foreigner Physical Examination Form: the medical record asked for at application and enrolment

The Foreigner Physical Examination Form is a medical record printed by Chinese quarantine authorities. Item 12 of the document list in the Application Guide for Chinese Government Scholarship asks scholarship applicants for a photocopy of it. Article 39 of Order No. 42 calls the record 外国人体格检查记录 and sets out the procedure international students follow with it at enrolment.

What the official text says

Item 12 of the Chinese Government Scholarship document list

These conditions are set out in the Application Guide for Chinese Government Scholarship, published by the China Scholarship Council (Campus China). They are conditions of that scholarship.

  • An applicant to study in China for more than six months shall submit a photocopy of the Foreigner Physical Examination Form.
  • The original is printed by Chinese quarantine authorities, is filled in in English, and is kept by the applicant.
  • The medical examinations must cover all the items listed in the form.
  • Incomplete records are invalid.
  • Records without the signature of the attending physician, the official stamp of the hospital, or a sealed photograph of the applicant are invalid.
  • Medical examination results are valid for only six months.

Item 12 does not say from which date the six months run.

Article 39 of Order No. 42

Order No. 42 is the Measures for the Administration of Schools Recruiting and Educating International Students, issued by the Ministry of Education, the Ministry of Foreign Affairs and the Ministry of Public Security. The English below is this site's translation of the Chinese text.

Article 39. International students shall, on enrolment, go to the Chinese health quarantine authority to complete the confirmation procedure for the Foreigner Physical Examination Record, or undergo a physical examination, in accordance with the rules of the Chinese health administrative department. Where the examination confirms a serious mental disorder, infectious pulmonary tuberculosis, or another infectious disease that may cause major harm to public health, as provided in the Exit and Entry Administration Law, the public security authorities handle the case in accordance with law.

Article 39 sets no validity period for the record.

Health certificate under the Regulations (separate document)

Article 16 of the Regulations of the People's Republic of China on Administration of the Entry and Exit of Foreigners (State Council Decree No. 637) names a health certificate for residence permit applications. It states:

When applying for a residence permit valid for more than 1 year, a foreigner shall, in accordance with relevant provisions, submit his or her health certificate. A health certificate is valid for six months beginning from the date of issue.

Article 16 does not name the Foreigner Physical Examination Form.
